For Skilled Nursing

When Safe & Successful Discharge is Impossible

Patients whose financial situation or care needs prevent safe and/or successful discharges can become full-time “residents.” These patients would benefit from a lower level of care but because of financial or other limitations, discharge is difficult… The result? They become a drain… on staff, on resources, on the balance sheet.

Financial Pathways to Safely Discharge Long Term Medi-Cal Patients

Master•Care is a credentialed Medi-Cal provider of Enhanced Care Management (ECM) and Community Supports (CS) under the new CalAIM program that launched in 2022.

January 2023, throughout California (with specific Managed Medi-Cal Plans) “Nursing Facility Transition / Diversion to RCFE / Home” was launched. With it, we can help you and your team safely-and-successfully discharge patients who may otherwise have limited financial and/or care options.

This is not the ALW program

Under this new program, we have access to a large database of RCFE and other providers, as well as pathways to pay for care that were previously unavailable.
